Treatment

If the angina pectoris always occurs in connection with the same type of stress, it is known as stable angina pectoris, which responds well to drug treatment. Nitrates are amongst the tried-and-tested substances to improve blood flow. Numerous studies have shown that a combination of drug treatment and regular physical exercise has fewer complications than bypass surgery. This is why it is advisable for most patients with stable angina pectoris to be treated first with drugs.

Nitroglycerine in the form of a nitrospray is often used. It acts within a few minutes by widening all the blood vessels in the body, which takes the pressure off the heart. The direct result of widening the coronary arteries is a better supply of oxygen to the heart muscle.

So you should always carry a nitrospray with you for emergencies. Surgical interventions may also be necessary. Examples are a bypass operation or ‘balloon’ dilatation that widens narrowed arteries. In many cases, arterial supports (stents) are also inserted.

Get your heart going!

Examples of measures to help your heart:

  • Cut out possible risk factors, such as stress, smoking, severe overweight, high blood lipid levels, high blood pressure, lack of exercise and extreme mental stress.
  • Take regular exercise! This trains the heart muscle, enabling it once again to perform its tasks better and gradually normalising blood pressure. Before you set up a programme of exercise for yourself, consult your doctor first. 
  • Carefully choose an appropriate form of physical exercise which you enjoy and which doesn’t put you under additional stress - for example, walking, hiking, cycling, swimming, gymnastics or light gardening.
  • Find people in a similar situation to exercise with. Exercise is often more fun with another person or in a group – it’s just not so much effort that way.
  • Don’t put yourself under any pressure. Instead, set yourself realistically achievable goals.
  • If you are severely overweight, reduce it gradually by consciously improving your diet. But do not stress your heart by dieting too rapidly, and follow a balanced and low-fat regime.

If your doctor has prescribed a nitrospray for you, always keep it with you. It widens constricted arteries and helps to shorten the duration of an angina pectoris attack. If necessary, you can also use your nitrospray as a preventative measure before strenuous exercise or stress that might otherwise trigger an attack.

 

 
Baldurat® – Active ingredient: dry extract of valerian root (3 - 6 : 1) [extraction agent: 70% ethanol (V/V)]. Nitrolingual akut® Spray – Active ingredient: glyceryl trinitrate. Composition: 1 metered dose oromucusal spray contains 0.4 mg glyceryl trinitrate. Other ingredients: medium-chain triglycerides and partial glycerides, ethanol anhydrans, peppermint oil. Indications: prophylaxis and treatment of acute attacks of angina pectoris, acute myocardial infarction, acute left-ventricular heart failure, catheter-induced coronary spasms. Contraindications: hypersensitivity to any of the ingredients or other nitrate compounds, acute circulatory failure, cardiogenic shock, marked hypotension (syst BP < 90 mmHg), intake of phosphodiesterase inhibitors for the treatment of erectile dysfunction or pulmonary arterial hypertension. Caution in cases of hypertrophic obstructive cardiomyopathy, constrictive pericarditis, pericardial tamponade, low filling pressures (e.g. in cases of acute MI; avoid syst BP < 90 mmHg), aortic and/or mitral stenosis, orthostatic circulatory disorders, increased intracranial pressure. During pregnancy and breast-feeding, only after careful consideration whether the benefits outweigh the risks. Side effects: headache very frequent at the start of treatment, frequent hypotension with reflex tachycardia, drowsiness, dizziness and weakness. Occasional nausea, vomiting, flush, allergic skin reactions, marked decreases in blood pressure with exacerbation of angina pectoris symptoms, bradycardia with fainting. Very rare, exfoliative dermatitis. Development of tolerance and cross tolerance possible, avoid continuous high dosing. Hypoxaemia and ischaemia may occur in CHD. GeloMyrtol® forte – Active ingredient: distillate of a mixture of rectified eucalyptus oil, rectified sweet orange oil, rectified myrtle oil and rectified lemon oil (66:32:1:1) (Myrtol® standardized). Indicated for acute and chronic bronchitis and inflammation of the maxillary sinuses (sinusitis). Nitrolingual® infus. – Active ingredient: glycerol trinitrate. Composition: 1 ml infusion solution contains 1 mg glycerol trinitrate. Other ingredients: glucose monohydrate (Ph.Eur.), 10% hydrochloric acid solution (for pH adjustment), water for injections. Indications: severe angina pectoris, acute myocardial infarction, acute left-ventricular heart failure, hypertensive crisis with acute heart failure, controlled hypotension. Contraindications: hypersensitivity to any of the ingredients or other nitrate compounds, acute circulatory failure, cardiogenic shock, toxic pulmonary oedema, marked hypotension (syst BP < 90 mmHg), disorders with increased intracranial pressure, intake of phosphodiesterase inhibitors for the treatment of erectile dysfunction or pulmonary arterial hypertension. Caution in cases of hypertrophic obstructive cardiomyopathy, constrictive pericarditis, pericardial tamponade, low filling pressures (e.g. in cases of acute MI; avoid syst BP < 90 mmHg), aortic and/or mitral valve stenosis, tendency to orthostatic circulatory disorders, severe liver or kidney disturbances.
